Acute Clinical Assessment and Diagnostic

Service Description

The service will provide comprehensive acute clinical assessments and diagnostic triage for patients presenting with urgent healthcare needs within the community setting. Key functions include: Initial patient assessment and clinical evaluation. Early identification of patients requiring escalation of care. Utilisation of the National Early Warning Score (NEWS2) assessment tool to identify clinical deterioration. Direct referral and escalation pathway to NHS Accident & Emergency (A&E) departments for patients meeting critical care criteria. Collaboration with primary care providers, community healthcare teams, and NHS services to ensure continuity of care.
